This is a more general release/waiver form that is much better suited for using in genetic genealogy y-DNA surname projects. Based on feedback from the genetic genealogy community and my own realization that the prior form posted here was not the best form to use, I have updated this page. This is a general release/waiver form that is much better than the prior form. You could also throw in a sentence that by signing this form, you consent to the project coordinator publishing the y-DNA results (and insert any other results, such as pedigree/ancestor information) on the internet (or use the term that we use in the publishing world: "or in any media").
Your use of the form acknowledges that you have read the Legal Notice at the bottom of this page. The legal notice is not a part of the form.
FOR GOOD CONSIDERATION, the undersigned jointly and severally forever release and discharge _____ [insert surname project coordinator's name] from any and all claims, liabilities, judgments, and proceedings, both at law and in equity, arising from my participation in the _____ [insert the project surname] SURNAME DNA PROJECT
This release shall be binding upon and inure to the benefit of the parties, their successors, assigns and personal representatives.
